Mailing Address Change Request

Filing this form will create a request to change your mailing address for assessment and property tax related notifications issued by the Assessor’s Office and Collector of Revenue’s Office. These mailings include but are not limited to your Change of Assessment Notice, Projected Tax Liability Notice, Personal Property Declaration, Real Estate Property Tax Bill, and Personal Property Tax Bill.

If you are changing your Personal Property address for 2026, please file your Personal Property Declaration online and make any address changes on the Declaration page. If you have already filed your Personal Property Declaration, please continue to the Mailing Address Change form.

If you do not have an existing active account, submitting an address change will not reinstate your account. Please schedule an appointment to have your account reinstated.
